How to request clinical documentation?

Clinical documentation of reports, tests or others can be requested at Parc Taulí as follows:

  • Copies of reports and additional scans can be collected 6 days later of the application.
  • For newly prepared follow-up reports, there is one maximum wait of 2 months. When the report is available, we will notify you by SMS al Mobile Phone* that you have provided to us at the time of requesting the documentation.

How to collect the documentation?

Accrediting identifying documentation that must be provided

The patient:

  • Original ID or passport.

Patient-authorized person:

  • Letter of authorization or representation signed by the patient that includes the full name of the authorized person and their relationship.
  • DNI or original passport of the patient. Attach a photocopy to the application.
  • DNI or original passport of the authorized person. Attach a photocopy to the application.

Dead

  • DNI or original passport of the deceased. Attach a photocopy to the application (if available).
  • Original DNI or passport of the applicant. Attach a photocopy to the application.
  • Documentation accrediting family ties. Attach a photocopy of the link (eg family book).

Legal patient tutor

  • Document accrediting the judicial appointment of a guardian
  • Original ID of the tutor. Attach a photocopy to the application.
  • Original DNI of the guardianship. Attach a photocopy to the application.

Parent in case of children under 16 years

  • Original DNI or passport of the applicant. Attach a photocopy to the application.
  • Family book. Attach a photocopy to the application.
