Antimicrobial Resistance: A threat to global public health

Antimicrobial resistance (AMR) is described as the ability of microorganisms such as bacteria, fungi, viruses and parasites to react against antimicrobial drugs, commonly through mutation, in order to render these ineffective –microbes are not inhibited and thereby they keep on multiplying and spreading. Thus, resistant strains of microbes emerge that are able to withstand the therapeutic effects of antibiotics, antiviral, antifungals and anthelmintics, amongst others. This natural phenomenon has been accelerated by the systematic abuse of antimicrobial drugs, and especially antibiotics, in the treatment of humans and animals.[1][2][3]

The growth and spread of AMR potentially hinders the ability to treat common infectious diseases, resulting in higher morbidity and mortality rates, and increased healthcare costs –patients infected with antimicrobial strain-resistant bacteria, viruses, etc. are more likely to die or to suffer from severe symptoms than those with non-resistant microbes, which increases the costs of treatment and patient management.[4]

It is estimated that by the year 2050, 10 million people would die annually of AMR-related causes, and the associated economic costs might rise to 100 trillion dollars.[5]     

The World Health Organization (WHO) has declared AMR as a serious threat to global public health and has called for a coordinated intergovernmental action for its prevention and management.[6] In response, initiatives and mechanisms have been adopted within the framework of the WHO, other intergovernmental agencies and national governments, as well as the private sector and NGOs, in order to promote and fund R&D on AMR.

In order to address AMR the European Comission has identified the following R&D areas:

Prevention, detection, effective infection control and surveillance
  • Interventions that prevent the development and spread of AMR.
  • Understanding the epidemiology of AMR.
  • New IT tools for early detection of resistant pathogens in humans.
  • New eHealth solutions improving prescription practices, self-management of health, care solutions, and improve awareness of AMR.
New therapeutics and alternatives
  • New antimicrobials and alternative products for humans  as well as the repurposing of old antimicrobials or the development of new combination therapies.
  • Digital technologies supporting the development of new therapeutics and alternatives.
New preventive vaccines
  • New effective preventive vaccines for humans and animals
Novel diagnostics
  • New diagnostic tools, and in particular on-site tests in humans.
  • Use of IT solutions in developing tools for diagnosing human infections.
Other areas of research
  • Knowledge gaps on the release of resistant microorganisms and antimicrobials into the environment and their spread.
  • New tools for monitoring antimicrobials and microorganisms resistant against antimicrobials in the environment.
  • New technologies that enable efficient and rapid degradation of antimicrobials in wastewater and the environment and reduce the spread of AMR.

The WHO has published a ‘Priority Pathogens List for R&D of new antibiotics’, classifying these into three categories:

Priority 1: Critical
  • Acinetobacter baumannii
  • Pseudomonas aeruginosa
  • Enterobacteriaceae
Priority 2: High
  • Enterococcus faecium
  • Staphylococcus aureus
  • Helicobacter pylori
  • Campylobacter
  • Salmonella spp.
  • Neisseria gonorrhoeae
Priority 3: Medium
  • Streptococcus pneumoniae
  • Haemophilus influenzae
  • Shigella spp.

Adding to this list, the European Commission establishes the priority of Tuberculosis, HIV/AIDS, Malaria and neglected infectious diseases.
