Introduction
The Corporació Sanitària Parc Taulí, with the commitment to providing quality healthcare service to its reference population, founded Parc Taulí Primary Care (PTPC) to meet the needs of the residents of Sabadell and other municipalities in the area of reference for primary care integrated with all the other facilities. PTPC has developed a unique model of primary care, centered on the citizen, coordinated with the municipal social work and healthcare teams, the active participation of Salut en Xarxa, in a shared management framework together with the Institut Català de la Salut and with the support of the Servei Català de la Salut. PTPC aims to integrate all the facilities and resources available to provide quality, integral, effective, personalized healthcare while promoting equality, continuity of care, satisfaction, efficiency, and sustainability.

Management model
Matrix organization
The primary care teams are organized by matricies to favor the development and motivation of the professionals making up the team. This organizational model enables the development of leaders among the professionals that can work across different teams, joining forces, making the organization flexible and adaptable, and making it possible to use all of the resources available at the different facilities in the town councils' municipal districts.

Coordination with other facilities
Primary care should fulfill the responsibility of coordinating the care process with all the healthcare and other facilities involved in the community's health, not only those related to sickness, to provide an integral, longitudinal vison and integral care that brings added value to the community's health. |
