  • Recreational and therapeutical activities:

    - Estimulació de la psicomotricitat fina- Stimulation of fine motor control
    - Memory stimulation
    - Cognitive stimulation
    - Workshops related to the preparation for certain holidays: Carnival, Christmas,...
    - Other crafts workshops to further socialization and integration with the environment
    -Summer camp
    - Attending spectacles, exhibitions...

 Who is eligible for the services?

  • Fragile persons with degenerative or chronic conditions and highly complex healthcare needs, with moderate dependence for activities necessary for daily life
  • Persons with degenerative or chronic conditions with high dependence for activities necessary for daily life
  • Severely handicapped young people
  • Persons with behavioral disorders (dementia, elderly persons with chronic psychiatric diseases, etc.)
  • Depending on the degree of dependency for activities necessary for daily life, they will be cared for in one of the following Social-Healthcare Units:

Unit for severely handicapped youths

  • Young people with traumatic neurological diseases
  • Persons that need help in daily life activities

 

Psychogeriatric unit

  • Patients with organic disorders and cognitive deterioration: dementia, Alzheimer
  • Patients with neurological disorders with congnitive deterioration
  • Patients with psychiatric disease with demential clinical presentations

 

Unit for highly dependent chronic patients

  • Adults with chronic diseases or multple pathologies
  • High therapeutic complexity
  • Persons that need help with most activities necessary for daily living
  • Relocation of residents from other units within the center

 

Unit for moderately dependent chronic patients

  • Adults with chronic diseases or multiple pathologies
  • Persons with a relative dependence for most activities necessary for daily living

 

Social-Healthcare Day Hospital

  • Patients with mild or moderate dependency that require assistance during the day to complement the therapeutic process (including physical, psychological, and social aspects)
  • Patients requiring convalescence and/or rehabilitation
  • Patients with chronic diseases that need their symptoms controlled
  • Patients with chronic diseases whose families have temporarily renounced them

Day Hospital for Persons with Dementia

An alternative to institutionalization that offers therapeutic care to persons with dementia and to their families, with the aims of rehabilitation, prevention, and integrated care

Exclusion criteria for the units

  • Active psychiatric disease
  • Dependency on toxic substances
  • Specific for the Social-Healthcare Day Hospital, in addition to those mentioned above:

- Highly dependent patients
- Patients with very advanced dementia
- Patients lacking family support

Objectives of integral geriatric evaluation

  • To correctly evaluate elderly persons from our reference area
  • To improve the identification of geriatric diseases and syndromes, especially of reversible situations
  • To ensure the proper use of the different levels of care
  • To keep persons in their own homes as long as possible with the best quality of life, delaying institutionalization
  • To design an individualized and continual care plan for each person
