Day Hospital for Persons with Dementia |
An alternative to institutionalization that offers therapeutic care to persons with dementia and to their families, with the aims of rehabilitation, prevention, and integrated care
|
Exclusion criteria for the units
- Active psychiatric disease
- Dependency on toxic substances
- Specific for the Social-Healthcare Day Hospital, in addition to those mentioned above:
- Highly dependent patients
- Patients with very advanced dementia
- Patients lacking family support
Objectives of integral geriatric evaluation
- To correctly evaluate elderly persons from our reference area
- To improve the identification of geriatric diseases and syndromes, especially of reversible situations
- To ensure the proper use of the different levels of care
- To keep persons in their own homes as long as possible with the best quality of life, delaying institutionalization
- To design an individualized and continual care plan for each person
