Parc Taulí has been selected by the Catalan Health Service (CatSalut) to develop two of the eight transformative projects that will be carried out in Catalonia until 2026 and that are part of CatSalut's strategic plan.

The first project is about mental health and aims to create a digital platform for the real-time management of patient data. The aim is for all professionals to be able to access patient information in order to develop preventive actions against changes in patterns or behavior that indicate a worsening of the patient. In addition, patients with mild and moderate pathologies will be offered tools and material for remote consumption - chatbot, tele-therapy, access to self-help videos and podcasts - to speed up their recovery process. The director of Mental Health of Parc Taulí, Dr. Diego Palao, has qualified this project as "the beginning of a profound transformation of the current healthcare model that will allow more patients to be served and with better quality. The new model will allow us to anticipate and manage our patients in a preventive manner, minimizing the risk of relapse and reducing acute hospitalizations".

The second project is Home Care 4.0 and its objective is to improve current clinical practice to gain efficiency and patient experience. For this reason, a new model of integrated health and social care will be developed with SAD Home Care with technological and digital support components to offer continuity of care at home for the elderly, improving well-being as a patient. The Director of Operations and Projects of Sabadell Gent Gran, Pedro Cano, emphasizes that "the main objective is to speed up hospital discharges, reducing income due to lack of social support and promoting home hospitalization with attention focused on the needs of the person and of a technological nature, which improves health outcomes, autonomy, well-being, satisfaction (patients and professionals) and the efficiency of resources".

With the development of these two projects, the Consorci Corporació Sanitària Parc Taulí continues to work to be a reference in the field of health and social innovation.
