Palliative care is a type of care offered to a patient with a advanced chronic disease, regardless of their age, and to their family with the aim of minimizing their physical and psycho-emotional suffering in end-of-life situations.

From'Intermediate Care Area of Parc Taulí are catered for 1736 adult palliative patients per year between the areas of hospitalization, home care and external consultations. In order to guarantee global care for the palliative patient, there is a comprehensive system based on the joint work of themultidisciplinary team in which the medical, nursing, social and psychological work team converge.

On the one hand, from Functional Interdisciplinary Sociosanitary Unit (UFISS) for Palliation the assessments of patients admitted to the acute hospital are carried out and the transfer to the Hospitalization Unit of Palliative Care, which has 21 beds, or other units.

On the other hand, there are three Palliative Support Team Home Care Programs (PADES)., which provide support together with Primary Care to patients treated at home. It is an active program every day of the year for 12 hours a day. Likewise, there is theOutpatient Comprehensive Assessment Team (EAIA) for Palliation who assesses outpatients.

"In recent years, the degree of complexity of patients has increased. For this reason, from the different devices we actively deal with situations that we consider to be reversible; we no longer only work for the end of life”, explains Teresa Ribot, head of the Palliative Care Unit.

 

El Parc Taulí, leader of a multicenter palliative care project

Parc Taulí is the entity executing the only strategic palliative care project of the Catalan Health System.

The aim of the multicentre project is to guarantee accessibility to a standardized palliative care model for all people with advanced chronic disease in the hospital environment. For this reason, work is being done on the development of a digital platform based on artificial intelligence and machine learning to improve the identification of these people, currently insufficient and based mainly on the experience and intuition of professionals. This fact will allow more people to benefit from a specific, standardized and excellent care model.

"From the Geriatrics and Palliative Care Service we promote accessibility to these specialized services to end what today is still a source of inequity in the system”, explains Doctor Marc Moreno, Director of the Geriatrics and Palliative Care Service and Project Promoter.

 

XII Congress of the Catalan-Balearic Palliative Care Society

The Fira de Sabadell will host between 21 and 22 February 2025 the thirteenth edition of the Congress of the Catalan-Balearic Palliative Care Society. This year, Parc Taulí is part of the organizing and scientific committee.

These are sessions aimed at care professionals whose aim is to create synergies with patient and family associations and present innovative experiences in the field of palliative care. Professionals interested in attending can now formalize your registration.

 

World Palliative Care Day

World Palliative Care Day is celebrated on the second Saturday of October, this year it is October 12. This day aims to promote awareness and understanding of the needs of people living with a disease that limits their day-to-day life and that of their families.
