Success of the II Primary Care-Hospital Care Conference of the Vallès Occidental Est (VOCE) 1080 720 Parc Taulí current affairs

The Parc Taulí Auditorium hosted this morning the 2nd Primary Care and Hospital Care Conference of the Vallès Occidental Este (VOCE), which welcomed professionals from both areas to share experiences and work to consolidate this coordination.

This coordination project between AP and AE to improve patient care has been the most outstanding and repeated in the institutional inauguration. The president of the CCSPT, Dolors Costa, has assured that “this day consolidates this coordination between the different professionals to guarantee more comprehensive and efficient areas of care”. Following the same line, the director of the Vallès Occidental est health sector, Ricard Armengol, has recalled that “primary care is the gateway for patients” in the health system and “it is important to coordinate primary care with the hospital environment to improve people's care. As well as establishing a respectful collaborative relationship between healthcare professionals in both areas for the good of the patient. We treat people, not diseases. We are obliged to have a holistic vision”. For her part, the manager of Primary Care of the Vallès Occidental and Oriental, Norma Henríquez, has insisted on the fact that "beyond the protocols, there are people. The relationship of the PA or the AE goes beyond the protocols. It is esteem, admiration, mutual respect. We work for people". The general director of the CCSPT, Gemma Craywinckel, has encouraged professionals "to continue working along the same lines" because "this formula of coordination and vision is what works".

The day began with a presentation of the current state of the relationship model between primary care and hospital care by the strategic director of APiC of the Department of Health, Rafa Ruiz. This was followed by a table where the care director of Parc Taulí, Dra. Carmen Sanclemente, explained how this coordination works. This first block closed with a table of experiences of this coordination in the field of hospital care from the PA; ENT nurse consultation for the study of hearing loss; the complex care project and the coordination of the PA and AE in rheumatology.

The second block began by explaining how primary care and hospital care are coordinated from a systems perspective and ended with new coordination experiences that have been nominated for the Best Coordination Project Grant for 2026.

The day ended with a keynote session by the director of Digital Strategy and Data at Sant Joan de Déu Hospital, Dr. Yolima Cossio, and the awarding of the Grant for the best coordination project of 2025 to Dr. Rebeca Gracia, psychiatrist at the Child and Adolescent Mental Health Service at Parc Taulí, and Dr. Ana Moreno. Both presented the project “Child Psychiatry Care Coordination in Patients with ADHD”.
