Interview with Marta Fernández Ros: Care manager.

 

After Margarida Forrellat came Mercè Ruiz and Pilar Bescos. All of them contributed a part of their personality to the Home Hospitalization project. "La Pili", as we know her at the Hospital, paved the way for us to discover how other HaDs in the country worked. With it we matured and laid the pillars of the current HaD which is a reference for other Home Hospitalization units in Catalonia, Spain, Europe and the world.

For six years now we have been fortunate enough to have Marta Fernàndez as our Care Manager. A great professional and a great person, an advocate of the crucial role that HaD nurses have. Aware that we are Advanced Practice Nurses, she has been a part of and supported us in nursing research projects and disseminated our role in national, international, multidisciplinary and specialized congresses.

Better ask her how she views her stay at HaD:

Marta, you are the Gcare mat from the HaD unit for six years now, what surprised you the most? d'this care method?

What surprised me most was the autonomy of the care team and that of the patient and the family itself. It must be said that this autonomy of the family / patient is the result of the involvement of nursing in the education of the patient and the family itself, a feedback that is created between all that makes this nursing-patient-family union unique.

Health education is very important in this service as patients are educated to have more autonomy and themselves learn to take self-care and be skilled in detecting signs and symptoms of any complication of their condition. of health.

Now, what would you highlight about this hospital admission system?

Home hospitalization is the best solution for continuity of care for patients who can continue their care and intravenous treatments at home, in their area of ​​comfort, we must not forget that for any of us the hospital does not stop being a hostile place, and if in addition to continuing home treatments autonomously I have a referral team that is in contact with the referral team of the hospital, it is a great opportunity.

You are the professional referent which evaluates surgical patients for admission to HaD. What are the pathologies? more frequent they suffer these patients?

Well, as you know we are a reference hospital for major surgical pathologies, we carry out complex care of all specialties: vascular surgery, breast surgery, trauma, otorhinolaryngology, maxilla, general surgery… drainage control, plastic surgery care, we have extensive training in the most innovative techniques in care. We monitor these until we assess that these cares are either discharged or will continue in primary school.

When you offer a deposit in the HaD What are the most frequently asked questions patients ask?

Doncs sincerely people have doubts, but what do they do to me outside? But who will come? I have to go to the head?

It is very important to evaluate the patient and his environment, since for us the most important thing is to create a level of trust with our team, it is our most valuable tool.

We are still a multidisciplinary team from the hospital that we move home to offer the same care that you would receive in the hospital but that will be taken to your home.

For that and very important to publicize our service not only to patients and relatives but to fellow professionals in the profession.

Sand the patient has already been admitted to HaD what is your most common opinion?

Just to tell you that it is often the patients themselves who ask their referring doctors for admission with us. They know they will be at home, with their comforts and their family, and their home hospitalization team will be on hand to oversee their care process and in ongoing communication with their referral hospital medical team.

They have already done so, and they are very happy with the service that has been given. We must not forget that we are all potential patients, and knowing that there is a service that offers you the care of a multidisciplinary team at home gives a lot of security.
