Law 42/2010 on health measures against smoking establishes a total ban on smoking in health centers and throughout their premises.

Parc Taulí has ​​been a member, since 2005, of the Catalan Network of Smoke-Free Hospitals (XCHSF), part of the European Network Smoke-free Hospital, promoted by the European Commission.

Both patients and professionals at Parc Taulí who are smokers can take advantage of the programs and aids for smoking cessation offered by the Corporation.

The Catalan Network of Smoke-Free Hospitals carries out and evaluates the degree of implementation of the project in each center every 2 years, through the achievement of the quality standards of the European Non Smoke Hospitals (ENSH), which are the ones it must meet a health organization to be considered free of tobacco smoke.

In our institution, the Smokeless Park Taulí Promotion Committee is the one that promotes, energizes and evaluates the guidelines established by the ENSH:

Preserve the premises and the environment of the hospital free of tobacco smoke.
Mark hospital areas as free of tobacco smoke.
Carry out health promotion, making both workers and citizens aware of the risks of tobacco use.
Offer help to smoking patients and workers (triptych) to stop smoking.

 


Among the objectives and general lines of work that have been defined, and that must be deployed over the coming months, are the following:

  • Design and implement a comprehensive internal policy that encourages the participation of all professionals - health and non-health - of all units and services.
  • Preserve the premises and the environment of the hospital free of tobacco smoke.
  • Mark the areas of the hospital (internal and external) as smoking bans.
  • Raise awareness among both professionals and citizens about the risks of tobacco use, and offer tools to help them quit.

Help to quit smoking

Parc Taulí has ​​different programs to help quit smoking:

  • For patients admitted.
  • For professionals.
  • For CAP Can Rull patients.

Smoking cessation program for admitted patients

Admitted smoking patients who want to start smoking cessation during admission or who have difficulty staying abstinent may receive help from the care team. Nicotine replacement treatment will be offered during the admission days and with the discharge process your family doctor will monitor the detoxification process.


Scope
This program is aimed at all admitted patients who show motivation to quit smoking or by prescription.

Indication of nicotine replacement therapy (TSN)

It consists of the administration of nicotine through a different route to cigarette consumption, in sufficient doses to avoid withdrawal syndrome and without creating dependence.

The basis of this treatment lies in the kinetics of nicotine in the body. After each puff, nicotine quickly reaches high concentrations in the blood reaching the brain in less than 9 seconds and stimulating neuroreceptors.

Various types of devices are used to supply nicotine: chewing gum, patches, mouth spray and sublingual tablets. None of these reach nicotine peaks as high as those obtained with cigarettes, although they do reach levels above 10ng / ml, which are sufficient to significantly decrease the intensity of the symptoms of the syndrome. of abstinence.

Nicotine patches - Mechanism of action

It is a transdermal nicotine dispensing system, consisting of a thin layer of adhesion to the skin, above which a nicotine reservoir appears.

Nicotine release is performed progressively, maintaining stable levels during the time of use.

The agreed day to quit smoking (Day D) is non-smoking, and the patch should be placed daily on a clean, dry, hairless area of ​​skin and the placement area should be changed daily (face anterior of the forearm, pelvis or thorax).

The patches currently marketed differ by the hours and amount of nicotine they can supply.

There are 24-hour release patches, which are kept on throughout the day, releasing doses of 21, 14 and 7 mg of nicotine; and 16-hour patches releasing doses of 15, 10, and 5 mg of nicotine, which are placed during the day. Both can be used in smokers with high, moderate or low dependence, varying the doses and time of use.

Indication of treatment according to dependence

The level of dependency must be easily assessed according to the following criteria:

  • LOW DEPENDENCE: Consumption <20 cigarettes / day or time between getting up in the morning and smoking the first cigarette> 30 minutes.
  • HIGH DEPENDENCE: Consumption> 20 cigarettes / day or time between getting up in the morning and smoking the first cigarette <30 minutes.

Professional smoking cessation program


All smoking professionals who want to quit smoking or who have difficulty staying abstinent during their working day can contact the Occupational Risk Prevention Service (Occupational Health) to receive help through the existing program.

Smoking cessation program for patients at CAP Can Rull


It is necessary to contact the general practitioner and the nursing professionals, who will be able to advise you in the process of quitting smoking, can supervise the attempt and / or prescribe pharmacological treatment if they consider it indicated.

Parc Taulí has ​​an action protocol to follow in the event of detecting a user or professional in breach of Law 42/2010, which establishes a total ban on smoking in health centers:

Users

If a user is detected smoking, they will be informed of the obligation to put out their cigarette, if they are inside the building, or to leave the premises, if they are outside the buildings.

If the user does not want to collaborate, they must inform Security (extension 3000). The Security Staff will inform you, again, of the smoking ban. And if he reaffirms his attitude, he will be brought to the attention of the corresponding public security forces.

Patients hospitalized

If you detect a hospitalized patient smoking inside the buildings, you must act as follows:

  • Any professional who detects a patient who is smoking should inform him of the obligation to quit smoking.
  • If the person who detects the patient is a professional of the institution, he will bring this to the attention of the patient's care team, so that they can offer him a treatment to deal with his smoking.
  • If the Security staff detects the patient, either through the video surveillance systems or in person, they will inform the Customer Service (SAC).
  • The SAC will inform the user that their conduct violates the regulations, and the penalties that their actions may entail. At the time of carrying out the communication, the SAC will make the user aware of the possibility of requesting from their healthcare team (doctor/nurse) a treatment to deal with their smoking during their stay at Parc Taulí.
  • If the hospitalized patient requests this treatment, it will be the medical team that will prescribe the indication, and it will be recorded in the discharge, the report or the pre-discharge.
  • In the face of repeated non-compliance by a patient, the Parc Taulí Security Department will report the violation to the corresponding public security forces.

Professionals from Parc Taulí

  • If the professional is smoking inside the premises, he will be informed of the obligation to leave the premises.
     
  • If you refuse to leave the premises, or are a repeat offender, or are inside the building, the violation will be reported to the Human Resources Department, which will contact its institutional manager in order to apply the sanction they may to carry out its actions, as established in the corresponding agreement or internal regime.
