What is the Social Work and Volunteering Area of ​​Parc Taulí?

It is one of the services that the person and / or family may need to address and treat the socio-familial difficulties generated by the disease, both acute and chronic. The activity is developed by paying attention to the needs of people and their environment and incorporating the vision and values ​​of the institution.

The Area of ​​Social Work and Volunteering, made up of 20 professionals, is present in all the centers of the Corporation, except in the UDIAT, and depends on Nursing Management.

What is social work?

The mission of social work is to attend to the social and emotional support needs of the person and their environment, derived and / or generated from an acute and / or chronic process of illness in the field of health.

Any health problem also generates psychosocial discomfort. Therefore, it is necessary to have a broader vision, beyond the purely biological one, to establish a complete diagnosis and more adjusted to the reality of the person. It is what is called the concept of biopsychosocial health (WHO, 1947).

Thus, the approach is carried out from a comprehensive perspective, through interdisciplinary work, which guarantees continuity of care and bearing in mind the opinions, values ​​and beliefs of the person.

Social work within the framework of a health and socio-health institution must establish and agree on coordination mechanisms with other levels of care in the Health System and with the other Social Services institutions in the territory.

What do social workers do?

  • Assessment and diagnosis of situations susceptible to socio-familial risk
  • Contribution to interdisciplinary teams, of our social assessment in order to make a comprehensive diagnosis
  • Information and advice on rights, services, resources ... that are useful to the person to promote their autonomy
  • Support and help people to overcome difficulties, to maintain personal / family balance, to adapt to their social reality, to enhance their own resources, either with or without material, technical and / or help. human resources
  • Collaboration in the sensitization of health professionals on the detection of social aspects of the people cared for
  • Coordination with other devices and interrelation of the person with the outside world

Support and support

The social worker carries out a helping relationship and an accompanying action, in a moment of fragility of the person, through active listening, understanding and strengthening of their abilities to promote the process of change to improve their personal, social and family well-being and / or the resolution of the situation that worries him.

This aid process can be carried out with or without the need for external resources: material, technical and / or human.
This function must be shared with other professionals in the care team.

Prevention

The social worker as one of the professionals in human relations and social welfare, and working with the team, tries to advance and prevent marginalization, social exclusion, poverty and social vulnerability, phenomena produced by poverty structural development of developed countries and affecting citizen coexistence.

Information and advice

The social worker, after assessing the socio-family situation, guides and manages, if necessary, human and organizational resources, contributing to the administration of public liability benefits, their distribution and the protection of the most dependent populations, according to the rights recognized by the welfare state.

Coordination

Coordination is a specific tool within social work. In this sense, our profession is a pioneer as this tool is implicit in our work that helps us to make a complete social diagnosis.
Due to the specialization of centers, services and equipment, coordination has become an indispensable practice in the intervention of the social worker because clients go through the services of different levels of care with great ease.
Coordination is carried out intra-institutionally (members of the care team) and inter-institutionally, with professionals from:

  • Primary and / or specialized social care services
  • Health services in general
  • Other services of the municipality (schools, security forces, residences ...)
  • External services in the region and / or province

The team of the Social Work and Volunteering Area of ​​the CCSPT attends to all the people who are linked to it, such as patients admitted to the different hospitalization units, patients being followed up by outpatients, patients incorporated into the different day hospitals. , in the Emergency Service ... All areas and services have a reference social worker (you can see it below).

The origin of the demand for social intervention can have different channels:

  • At the request of the person and / or family.
  • By assessment of the care team.
  • For indicators of social risk and / or specific social intervention programs.
  • Coming from external institutions.

The social workers of the Home Care and Support Team Program (PADES) and the Interdisciplinary Sociosanitary Functional Unit (UFISS) do not have a visit scheduling agenda. To request an appointment, you must contact them directly and / or through the care team that attends to the person.
