The aim of the day is to improve care coordination between primary and specialized care, for the benefit of the renal patient.

Under the motto 'Joining forces to improve care for kidney patients' will be held next Wednesday, March 2, in the Taulí Auditorium, a day of updating knowledge and coordination between professionals of the Nephrology Service of the Hospital de Sabadell and the Primary Care Centers of the reference area of ​​the Parc Taulí. The day, organized in morning and afternoon edition with the same contents, more than 300 professionals have registered, including doctors and nursing professionals.

In our reference area it is estimated that there are 30.685 citizens with chronic kidney disease, 10% of the general adult population. The prevalence recorded is only 4.424 patients with a diagnosis, a figure that corresponds to 1,3% of the population. It is estimated that about 2.320 patients have advanced chronic renal failure. Many of them will end up needing dialysis and / or kidney transplantation. Consequently, there is a lot of hidden kidney disease.

El Nephrology Service of Sabadell Hospital offers a wide range of quality care coverage to the population, and has the largest public hemodialysis unit in Catalonia, with nearly 150 patients receiving assisted hemodialysis, a life support treatment for people suffering from kidney failure very advanced. On the other hand, the Home Peritoneal Dialysis Program treats about 30 patients a year at home.

The activity of kidney transplantation in the reference units of Barcelona is extensive, and during 2015 19 patients from Parc Taulí received a transplant.

The care of acute renal patients is completed in the different care devices: Emergencies, Hospitalization, Intensive Care, Day Hospital and Outpatients. One of the objectives of the Nephrology Service is coordination with Primary Care, which aims at prevention, early diagnosis and the start of treatment in the renal patient in our area. To improve this coordination, work is being done within the framework of the so-called 'Route of Chronic Kidney Disease' (MRC) of the 2015 CatSalut Health Plan, which establishes how healthcare professionals in the area are organized to apply the best practice. clinic.

The main objective of the day on March 2 at Parc Taulí is to publicize the route of chronic kidney disease and, from the discussion of very practical clinical cases, resolve doubts of complex cases and explain the circuit that is should follow in optimal care for the kidney patient.
